A Colorado voter registration form must be completed by residents of Colorado who wish to vote in any upcoming local, state or federal election. Colorado citizens are able to register at the age of sixteen (16), but they must wait until they are eighteen (18) years of age to cast a vote. The registration process can also be completed online through the website posted below. Online registration will require a Colorado State driver’s license or an ID card issued by the Department of Revenue. Voters can register all the way up until election day, but individuals who register less than eight (8) days before an election will have to visit a Voter Service and Polling Center in their county to receive a ballot. Registration completed through a registration drive must be submitted twenty-two (22) days before an election.

Voter ID (§ 1-7-110)- All voters must present a valid piece of identification at the polling station.
How to Write
Step 1 – Download the printable Colorado Voter Registration Form.
Step 2 – Provide the following information under sections (1) through (3):
- Citizenship status
- Full name
- Birthdate
- Valid identification number
Those without a valid ID number can tick the last check-box under section (3).
Step 3 – Under section (4), provide the voter’s address of residence. Section (5) allows individuals to enter a separate mailing address and section (6) provides the opportunity to enter a third address to which the voting ballot will be sent. Select the “Save as above” box if all addresses are one and the same.
Step 4 – At this juncture, voters must select the political party with which they are affiliated under section (7a). Those that are unaffiliated should select what type of ballot they desire under section (7b). Anyone updating their information must complete section (8). All registrars must sign and date the form in front of a witness, who must do the same.
Step 5 – Section (10) is optional; voters can use this section to provide a phone number, gender, and email address.
