An Idaho voter registration form is used to register to vote in the State of Idaho. All registrants must be at least eighteen (18) years of age and must have resided in Idaho for thirty (30) days prior to the election date. Registration forms can be submitted by mail or in-person to the voter’s local County Elections Office. This form should be submitted twenty-five (25) days prior to the election date. Voters may choose to register at the polls on election day, as long as they bring proof of ID and Idaho residency.

Voter ID (§ 34-1113)- Voters must present photo ID or sign an affidavit (§ 34-1114).
How to Write
Step 1 – Download the Idaho Voter Registration Form.
Step 2 – Registrants must provide the following information in the appropriate fields:
- Full name
- Driver’s license number or SSN
- Citizenship status
- Age affirmation
- Date of birth
- Residence address
- Length of Idaho residency
- Previously registered address (if applicable)
- Political party affiliation
- Previous registered name (if applicable)
- Legal disqualifications
- Signature
- Date
Step 3 – All forms must be mailed or hand-delivered to the registrant’s County Elections Office.
