New York Voter Registration Form – Register to Vote in NY

A New York voter registration form can be used by New York residents to register to vote in-person or via mail. An online registration option is also available on the New York government website. To be eligible to vote, one must be a U.S. citizens of at least 18 years of age (by December 31st of their registration year) who is not currently serving out a prison sentence or on parole, who is mentally competent, and who is not registered to vote in another state. The registration deadline for each election is 25 days before election day.

Where to Send Voter Registration Form – When submitting a voter registration form by mail, residents must send it to their county’s board of elections office.

Voter ID (§ 8-303)  – In New York, registered voters are not required to present ID when voting. However, if an individual’s identification has not been properly verified during the registration process, they may be required to present a valid photo ID or a document that shows their name and address (utility bill, bank statement, government check, paycheck, etc.).
