A witness incident report is a form that records an individual’s statement about what occurred during a specific incident. If, for example, a bystander witnesses an accident or a workplace injury, filling out a witness incident report can be helpful in any court or insurance claim decisions that may arise. The incident report asks for such information as the time and location of the incident, the person(s) involved, and any treatment administered, whether medical or otherwise.
