Oregon Revocation of Power of Attorney Form

An Oregon revocation of power of attorney is a document that terminates a previously delegated power of attorney. Before the revocation can become legally enforced, the principal must give notice of the cancellation to the delegated agent and to any entities or institutions that may be acting on the original power of attorney arrangement.

Financial Powers

A financial power of attorney terminates if:[1]

  • The principal dies;
  • The principal becomes incapacitated and the power of attorney is not durable;
  • The principal or the court terminates the power of attorney;
  • The power of attorney has an end date;
  • The purpose of the power of attorney is completed; or
  • The agent resigns, dies, or becomes incapacitated or financially incapable.

Medical Powers

An advance directive can be revoked by the principal at any time by:[2]

  • Expressing the intent to revoke in any manner, such as in writing or verbally; or
  • Executing a new advance directive.
