Pennsylvania Revocation of Power of Attorney Form

A Pennsylvania revocation of power of attorney is a document used to revoke a previously created power of attorney appointment. In addition to filling out this form, the principal must provide notice to the authorized agent and to relevant financial and medical institutions of the revocation. Otherwise, it is not considered legally binding.

Financial Powers

A financial power of attorney becomes terminated if:[1]

  • The principal provides a written notice of revocation;
  • The power of attorney has an expiration date; or
  • The agent is notified about the principal’s death or incapacitation, and the POA is non-durable.

Medical Powers

A health care power of attorney can be revoked by the principal at any time while they are of sound mind. This can be done by:[2]

  • Writing and signing a revocation statement in the presence of two witnesses who are 18 or older; or
  • Personally informing the attending physician or agent.
