The Oklahoma Rental Application is a document used by owners/landlords/property managers to collect the necessary information to verify an applicant’s eligibility for tenancy on their property. This document will request information from the prospective tenant with regard to rental history, employment information and income, references, and other components required to make a proper decision with regard to their reliability as a renter. Credit checks and criminal background checks may also be performed and the landlord may charge a fee for these services.

Application Fee (No Statute) – There is no limit to the amount a landlord can charge for an application fee and they are non-refundable.

Security Deposit (No Statute) – There is no maximum amount the landlord may request for a security deposit.

How To Write

Step 1 – Section 1 – Landlord must complete the following fields in this section as follows:

  • Enter the Property Address
  • Lease Start Date in mm/dd/yyyy format
  • Lease End Date in mm/dd/yyyy format
  • Enter the Amount of the rent (paid in certified funds before or at the time of the beginning of possession of the rental)
  • Enter the Security Deposit amount( Check the preferred box as to preference of payment)
  • Enter, again, the security deposit amount
  • Enter the amount of the Application and Credit Check Processing fee
  • Enter the amount of days that the applicant will have to review the application before they are required to sign the application and pay the application fee

Step 2 – Applicant Information –  Enter the following:

  • Name (First, Middle, Last)
  • SSN
  • Date of Birth
  • Best Phone Number
  • Work Number
  • Cell Number
  • Email Address
  • Driver’s License Number
  • Photo ID (Check yes or no)

In the event of an emergency or death (pursuant to Title 41 O.S. §130.1A) notification is to be made to:) – Must not be anyone living with you nor any co-applicant

Step 6 – Pet Information – Provide the following information with regard to pets

  • Age
  • Neutered or Spayed (check yes or no)
  • Check whether your pet is an indoor or outdoor pet

Step 7 – General Information –

  • Will any smoker’s occupy the property?
  • Do you have Tenant’s Homeowner’s Insurance – If yes, name the insurance company

Step 8 – Vehicle Information – List all vehicles as follows:

  • Make and Model
  • License Plate Number

Step 9 – Applicant’s must carefully review all of the information in the last two paragraphs prior to signing the application. Once reviewed, understood and agreed to provide the following

  • Applicant’s Signature(s)
  • Date of Signature(s)
  • AND
  • Signature of the Broker who will acknowledge receipt of the non-refundable processing fee
  • Date the Broker’s Signature
