Duties and Responsibilities
- Develop client profiles;
- Establish treatment plans;
- Meet regularly with clients to assess needs;
- Document treatment and response;
- Discuss progress with healthcare providers;
- Coordinate with insurance companies;
- Research alternative treatment options; and
- Check-in with clients post-discharge.
Qualifications
- Degree in nursing or related field;
- Patience with bureaucracy;
- Empathetic personality;
- Extremely organized;
- Familiarity with computer-based case management systems; and
- Understanding of insurance/billing protocols.
