Vehicle Registration Forms
- Bill of Sale;
- Odometer Disclosure Statement (Form TC 96-5);
- Completed Application for Kentucky Certificate of Title/Registration (Form TC 96-182);
- Valid Kentucky Driver’s License and Social Security Number;
- Vehicle Inspection from County Sherrif’s Office (only needed if the vehicle was transferred from out of state);
- Registration Fees (rates may vary);
- Proof of Insurance within 45 days with the following minimum requirements:[1]
- $25,000 for bodily injury, per person;
- $50,000 for total bodily injury, per accident;
- $25,000 for property damage;
- Motor Vehicle Power of Attorney, if an agent shall represent the vehicle owner in any licensing or titling matters.
Boat Registration Forms
- Bill of Sale;
- Completed Motor Boat Transaction Record Application for Title/Registration (Form TC 96-184);
- Manufacturer’s Certificate of Origin (only for brand-new boats);
- Signed Certificate of Title (if lost or damaged, see Replacement/Duplicate Kentucky Titles)
- Photo or Pencil Tracing of Hull Identification Number (for vessels manufactured after 1972);
- Valid Kentucky Driver’s License or Proof of Identity; and
- Registration Fees (rates vary depending on the vessel’s classification).[2]
