The Massachusetts General Bill of Sale Form is generally a form that is used between two private parties to prove the legal sale and acquisition of any kind of personal item(s). This document will provide information and descriptions needed with regard to the seller, purchaser and item(s) being sold. This form will require a notary public witness at the end, therefore any signing parties should withhold signatures prior to retaining the services of a notary and all parties must be present.

How to Write

Step 1 – Seller and Buyer Information – Seller and Buyer must complete the following:

  • The Seller –
  • Name
  • Driver’s License Number
  • Physical Address
  • City
  • State
  • AND
  • The Buyer –
  • Name
  • Driver’s License Number
  • Physical Address
  • City
  • State

Step 2 – Property Details – Describe the details of the property being sold and purchased in the lines provided:

  • Details
  • Serial Number (SN) (if any)

Step 3 – Trade/Purchase Price – Check the boxes that apply and provide additional information if required:

  • If the first box is selected, the seller will only accept a monetary transaction – Enter the dollar amount in words
  • Enter the dollar amount in numbers
  • Provide a date as to when when the parties have agreed that payment will be made – mm/dd/yyyy format
  • Enter the date in which the document will be known in mm/dd/yyyy format
  • If the parties have agreed that payment will be made at a later date, enter that date mm/dd/yyyy format
  • Other – If different negotiations have been agreed upon by the parties, enter a brief description (if extra room is needed add a sheet)
  • Check the box  if item(s) are being provided as a gift.
  • The last box in this section will address a direct trade between seller and buyer. Enter a brief description of the agreed terms of the trade

Step 4 – Seller’s Disclosure – The seller must read the statements and provide information regarding any known defects

  • If all signatories are present before a notary public and have been identified the seller must sign in agreement to the statement after recording any defects (if applicable)

Step 5 – Signatures – If all parties are present before a notary public provide the following:

  • The Seller –
  • Signature
  • Printed Name
  • AND
  • The Buyer –
  • Signature
  • Printed Name
  • AND
  • The Witness – 
  • Signature
  • Printed Name

Step 6 – Notarization – As soon as the notary has witnessed all signatures, the notary public, will then complete the remainder of the form, also affixing  their state seal.

  • Make copies of the document to provide to all who signed, for their records
  • Give the original copy to the buyer
