1. Services (what is being maintained)
Define what is being maintained and any scheduled updates (unless what is being maintained is taken care of when things fail). This should be clearly defined to ensure the company’s responsibilities.
MAINTENANCE. The Company agrees to provide the Client maintenance services for the following: __________________________________________ (“Services”).
The Services shall be provided: (check one)
☐ – On a Recurring Basis. The Company agrees to provide the Services each ☐ week ☐ month ☐ other: __________________ as part of this Agreement.
☐ – When Needed. The Company agrees to provide the Services when needed or as requested by the Client.
2. Materials and Labor Costs
Routine maintenance will generally cover expected repairs that have fixed costs. Any special requests or non-standard maintenance items will be charged at the cost of the company.
INCLUDED SERVICES. The Company agrees to provide standard maintenance items in accordance with the described Services. Payments made by the Client: (check one)
☐ – Shall Include BOTH Labor and Materials Costs. The Services provided by the Company shall include all maintenance items required to repair or satisfy the request. Any outstanding repairs needed, no matter its cost, shall be the Company’s responsibility. Any special or custom requests will be charged separately.
☐ – Shall Include ONLY Labor Costs. The Services provided by the Company shall include labor costs only. Any products or materials required as part of the Services shall be billed separately to the Client.
3. Retainer
To ensure the client’s commitment to the agreement, a retainer or partial payment for future services may be required by the company. It is recommended, although not required.
RETAINER. As part of the Client’s commitment to this Agreement, the Company: (check one)

4. Payments
