The Ohio Three (3) Day Notice to Quit (Non-Payment of Rent) is used when a party, typically a landlord or owner, notifies the opposite party, the tenant, to leave the premises due to their failure to pay rent on the due date. The tenant then has three (3) days to remedy the past-due rent after they receive the notice to quit.

Delivery should be made by handing a written copy of the notice to the defendant personally, or if the tenant is unavailable to leave it at the tenant’s usual place of abode. Also mailing a copy by certified mail with return receipt will work too.

How to Write

Step 1 – The first 3 fields are used to enter the tenant’s name(s). If there is only one tenant, leave the other two fields blank.

Step 2 – The paragraph following describes that the tenant must leave the premises if rent is not paid within 3 days of this service. Enter the address and include the Township and County.

Step 3 – Enter the amount of rent the tenant owes including any fees or other charges.

Step 4 – Lastly, date and sign the document. Enter your, the landlord’s, contact address and telephone number.
