A student incident report includes a description of an event that occurred while at an educational institution. The report will give an accurate depiction of the incident and all those involved (including what caused the event). All witnesses and statements should be included that describes the behavior and actions of the student.
After completing, the incident report should be filed with the school where it will be kept confidential.
May be used for
- Concerning or unusual behavior;
- Gambling activities;
- Sexual assault;
- Sexual harassment;
- Threats (verbal or online); and
- Violent acts.
