Passport Renewal Application (Form DS-82)

A passport renewal application (form DS-82) is used to renew an existing or recently expired U.S. passport book and/or card, which serves as a travel document verifying the identity of U.S. citizens or non-citizen U.S. nationals.

DS-82 Requirements

All of the following must be true to use this form:

✓ I can include my current U.S. passport book and/or card along with this application.

✓ I was 16 years old or older when my most recent U.S. passport book and/or card was issued.

✓ My most recent U.S. passport book and/or card was issued within the last 15 years.

✓ The U.S. passport book and/or card I am renewing is in good condition and has not been mutilated, damaged, reported lost, or reported stolen.

✓ My U.S. passport has/had a full validity period of ten years and was not restricted due to passport damage, multiple thefts or losses, or non-compliance with 22 C.F.R. 51.41.

✓ There have been no changes to my name since issuing my most recent U.S. passport book and/or card (unless you can provide certified documentation to support the name change).

How to Renew a U.S. Passport

You must include your most recent passport book and/or card when mailing your application.

  1. Complete and Sign Form DS-82
  2. Attach a recent photo (six months or less) taken by a professional passport photo service (Walgreens, CVS). Vertically staple the photo as close to the edge as possible, using the template at the bottom of page one (form ds-82).
  3. Calculate fees by using the U.S. State Department’s Passport Fee Calculator. Fees are non-refundable and should be payable to the “U.S. State Department.” Do not send cash.
  4. Mail application to one of the following:
    1. Routine Service for CA, FL, IL, MN, NY, or TX: National Passport Processing Center PO Box 640155 Irving, TX 75064-0155
    2. Routine Service for all other states and Canada: National Passport Processing Center PO Box 90155 Philadelphia, PA 19190-0155
    3. Expedited Serice for all states and Canada ($60 additional fee) – National Passport Processing Center PO Box 90955 Philadelphia, PA 19190-0955
  5. Track your passport renewal online via Passportstatus.state.gov.
  6. Wait and receive your new passport in the mail with attachments.

DS-82 Form Parts

There are a total six pages within form DS-82. The first four pages are instructions only. You must complete the last two pages, printed on two separate pages, in black ink.

      • Regular Book (standard)
      • Large Book (non-standard) – At no extra cost, we recommend selecting ‘large book.’

  1. Name (last, first, middle) – The name to be entered in the passport should match the name on your previous U.S. passport. If you are renewing your passport with a name different from the one on your previous U.S. passport, include evidence of a name change (marriage certificate, divorce decree, court order, etc).
  2. Date of Birth – In the format such as (MM/DD/YYYY)
  3. Gender – Select “Yes” if you are changing your gender from your previous passport.
    1. M (“male”)
    2. F (“female”)
    3. X (“unspecified or another gender identity”) – Warning: Other countries supporting only two genders may not recognize this gender option on your passport, resulting in denied entry.

How Long Does it Take to Renew a Passport?

The US Department of State, the government agency responsible for issuing passports, gives options for normal processing fees and faster (expedited) processing speeds. Additionally, you can purchase priority shipping to speed along the process even quicker.

  • Routine (normal) – 10 to 13 weeks on average, no charge
  • Expedited (faster) – 7 to 8 weeks but can be as quick as 5 weeks, +$60
  • Expedited at Agency (fastest) – 1 to 2 weeks but must show proof of international travel plans. Call 1-877-487-2778 to make an appointment, +$60

Instructions for Fast Delivery

  1. Add up the fees: Passport Book Renewal ($130), Expedite Fee ($60), Priority Mail Return ($19.53)
  2. Write a Check for $209.53 payable to the “US Department of State”.
  3. Priority Mail Express your completed application to the National Passport Processing Center.
  4. Receive your new passport in 5 weeks or less.
