4 – Only The Principal May Verify This Document And Execute It

The final statement of this document will contain three blank spaces. The Principal should enter the Current Day, Month, and Year he or she signs this form using these spaces. Below this statement, the Principal should Sign and Print his or her Name in the Presence of a Notary Public.
