An Oklahoma motor vehicle power of attorney form is a document that allows you to appoint another individual to handle your affairs with the Oklahoma Motor Vehicle Commission. These matters may relate to titling, registration or other transactions for which you need someone to represent your interest in a motor vehicle.

1 – Fill Out And Submit This Form To Grant Vehicular Power To An Agent

2 – Bring The Vehicle To A Service Station With This Paperwork

The second page will need to be submitted with the Vehicle to an inspection or service station. The technician there will verify the Vehicle’s Year, Make, Model, Style, Vehicle Identification Number, and Odometer Reading. In addition, the Mechanic or Technician will submit his or her own Name, Company, Site Address, and Credentials then inspect the Vehicle to document notable malfunctions or defects.

3 – The Vehicle’s Owner Must Make A Declaration And Self Report

The first page will be the official declaration of appointment the Principal wishes to make. Some information will be mandatory for this document to be effective. Begin by producing the Legal Name of the Vehicle Owner on the first blank space. Then, solidify the identity of the Agent being assigned Principal Power here by documenting it on the next blank line (after the words “Gives To”). The Principal Vehicle Owner or an Authorized Representative of the Business that owns the Vehicle must sign this document on the blank line following the words “Signed By”One of two areas will need to be satisfied to lock in the Principal’s Identity. Should the Principal be a person, then his or her Birth Date and Social Security Number should be furnished in the statement beginning with the word “NOTE: If This Power Of Attorney Is In An Individual’s Name.” The first area will require a DD/MM/YY format while the second statement will require the standard Social Security Number format (xx-xxx-xxxx)  If The Principal Vehicle Owner is a Business and that Business is issuing this Power, then supply its Federal Entity Identification Number on the blank space following the words “…Please Include Its Federal ID Number.”The remainder of the first page is supplied for the use of the Notary Public attending this signing. He or she will be the only party who can satisfy these requirements.
