Wisconsin Revocation of Power of Attorney Form

A Wisconsin revocation of power of attorney is a form used to terminate or cancel the powers granted to an authorized agent by a previous power of attorney. After executing this form, the principal must give notice to the agent and any entities that received the original power of attorney. Otherwise, the revocation is not legally binding.

Financial Powers

A financial power of attorney is canceled if:[1]

  • The principal dies;
  • The principal becomes incapacitated and the power of attorney is not durable;
  • The principal revokes the power of attorney;
  • The power of attorney has an end date;
  • The purpose of the power of attorney is completed; or
  • The agent resigns, becomes incapacitated, or dies, and the POA does not provide a successor agent.

Medical Powers

A power of attorney for health care can be revoked by the principal at any time by:[2]

  • Physically destroying the document;
  • Executing a written statement revoking the POA;
  • Verbally expressing the intent to revoke in the presence of two witnesses; or
  • Executing a new power of attorney for healthcare.
