An Epilepsy Foundation donation receipt is used to show proof of a contribution and is meant to be kept in the donor’s records of that tax year until it comes time to itemizing tax deductions. As a 501(c)(3) charity, the Epilepsy Foundation accepts donations such as clothing, cash, stocks and monetary items that go in support of the 3.4 million people in the US living with Epilepsy.

2 – Name The Donation Date

Naturally, interested Tax Entities will want to know the exact “Value” of the item being donated. This will be based on the expected price the donated item could potentially be sold for in a fair market. Print two copies of this paperwork, dispense one to the Donor, and retain one for your files.
