The Missouri Rental Application Form has been devised to be a useful tool a Landlord or Property Owner may employ to organize his or her search for a preferred Tenant to rent an available property. This form will have a definitive format which will call for the wealth of information that needs to be presented to a Property Manager regarding an applicant’s candidacy for a rental arrangement. The Property Manager’s top priority will be to safeguard the property while having a law-abiding Tenant who is not a nuisance to the community to pay his or her rent on a regular basis and in full. In order to find such a Tenant, Property Manager’s will require proof of income, references, and the ability to perform reference, background, and credit checks. This adds a financial burden to the Property Manager’s efforts which is typically passed on to the applicant.

In addition to the financial burden, reviewing applications is a time-consuming effort. This creates the need for an organized process which such an application will provide. The fee and consent line to perform are thus, vital parts of this form for as integral as they are to the process, a prospective candidate has the right to full disclosure that such requirements will apply to his/her efforts in applying to the property in question.

How to Write

For Office Worker/Landlord.

Step 1 –  In the box labeled “Brokerage Disclosure” enter the name of the Listing Broker on the first line and the name of the Leasing Broker on the second line. Then indicate if the Leasing Broker represents the Landlord by checking the box next to the word Landlord or represents the Tenant by check the box next to the word “Tenant.”

Step 2 – The Applicant must place his or her initials at the bottom of this box. The Leasing Agent named in this box must attach their business card to this application.

Step 3 – Below the “Brokerage Disclosure” box, the applicant must provide an Identification Number and an Expiration Date for this I.D. (i.e. a Driver’s License). Report this I.D. on the line below this box.

Step 4 – Under the heading “Conditions” will have three blank spaces. On the first blank space enter the non-refundable fee an applicant must pay to submit this application.

Step 5 – On the second blank space, under the heading “Conditions,” enter the required deposit amount a Tenant must submit with this application.

Step 6 – On the third blank line, under the heading “Conditions,” enter the name of the entity that shall hold the Applicant’s Deposit.

Step 7 – In the box at the bottom of the first page labeled “Office Use Only,’ enter the date and time the application has been submitted and received on the first line.

Step 8 – On the second line, enter the name of the individual who has reviewed the application.

Step 9 – On the third line indicate the Applicant’s status by placing a checkmark in one of the boxes (Accepted, Rejected, Withdrawn, Applicant or Agent Notified). Then enter the Date and Time this decision has been finalized.

For Applicant

Step 1 – Place your Initials at the bottom of the box labeled “Brokerage Disclosure,” if this box contains information regarding the identity of the Brokers involved.

Step 2 – Under the heading “Conditions,” locate the columns labeled “Contact Numbers: Applicant 1” and “Applicant 2.” Enter your Cell Phone number on the first line, your Home Number on the second line, your Work Number on the third line, and your Email on the fourth line. If another individual is applying with you, he or she must provide their contact numbers as well in the appropriate area.

Step 3 – Initial the bottom of the first page in the space provided in the lower right-hand corner. There is enough room for two applicants to provide their initials.

Step 4 – Under the heading “Applicants Agree and Understand That,” will be a few statements which must be read. These will be made up of disclosures and requirements for the Applicant. The bottom of this section will have a box with a paragraph in bold letters. This will be a verification statement and consent line. Any Applicant’s providing information on this application and seeking a residence must sign their names and date the signature on the space(s) provided.

Step 5 – The area below this will have two columns. One for the Applicant (labeled “Applicant 1”). The one on the right (labeled “Applicant 2”) is reserved in case there is a Co-Applicant. Both must provide the same information. Enter your name on the first line of the appropriate column.

Step 6 – On the second line, enter your Date of Birth and Social Security Number.

Step 7 – On the third line, enter your Current Street Address.

Step 8 – On the fourth line, enter the City, State, and Zip Code of your current residence.

Step 9 – On the fifth line, enter the Date you moved into Your current residence in the first blank space. On the second blank space, enter the date you will need to move out. Then in the third blank space, enter your monthly rent or mortgage payment and check the box to report which one this figure is (located directly below the space with the dollar sign.)

Step 10 – On the sixth blank line, labeled “Landlord/Management/Mortgage Co. Name,” enter the name of the entity you must pay your monthly rent to.

Step 11 – On the seventh blank line enter the Phone Number then the Email of your Landlord, Property Manager, or Mortgage Company.

Step 12 – On the eighth blank line, labeled “Reason for Moving,” report on why you must move out of your current residence.

Step 13 – Initial the bottom of the page in the bottom right-hand corner. If there is a Co-Applicant they must initial this area as well, after filling out the column labeled “Applicant 2.”

Step 14 – The third page will have a similar format as the second. There will be two columns, one for each applicant, that must be filled out. The first line will require the street address of your previous residence on the first line.

Step 15 – On the second line, enter the City, State, and Zip Code of your previous address.

Step 16 – On the third line, enter the Date you moved into your previous residence. Then enter the date you moved out of your previous residence. Finally, enter your monthly rent/mortgage payment and indicate which it is by placing a check mark in either the box labeled “Rent” or the box “labeled “Mortgage.”

Step 17 – On the fourth line, enter the name of the Landlord, Property Manager, or Mortgage Company you pay for your current residence.

Step 18 – On the fifth blank line, enter the Phone Number then the Email of your Landlord, Property Manager, or Mortgage Company.

Step 19 – On the sixth blank line, report the reason you moved from your previous residence.

Step 20 – Under the heading “Employment,” enter the name of the employer you worked for next to the number “1,” on the line labeled “Current Company Name.”

Step 21 – On the second line enter the Location of your employer, the date you started working in your current position, and the last date you worked there (if presently employed there, write the word “present”).

Step 22 – On the third blank line, enter the Title of the Position you hold then your yearly income.

Step 23 – On the fourth blank line enter the name of your supervisor and his/her contact number.

Step 24 – On the fifth blank line, next to the number “2” and labeled “Previous Company Name,” enter the name of your previous employer.

Step 25 – On the sixth blank line, enter the address of your previous employer. Then enter the date range you were employed for with your previous employer.

Step 26 – On the seventh blank line, labeled “Position/Rank,” enter the position you held with your previous employer. Then enter your yearly income there.

Step 27 – On the eight blank line, enter the name of the Supervisor you answered to at your previous place of employment.

Step 28 – Under the heading “Additional Income,” enter the source of any additional income you may have then the amount of money you earn per year.

Step 29 – Beneath the question “Do you have any animals?” there is a table where you may report the Type, Breed, Age, and Weight of your bed. In addition, there will be two columns to indicate if your pet is a Male or Female and if it has been Neutered or Declawed.

Step 30 – The next table will require you report some information regarding your vehicle. Enter your vehicle’s Type, Make, Model, State (issuing). There will be a space to the right of this where a Co-Applicant may enter information regarding his or her vehicle as well.

Step 31 – You must initial the bottom right-hand corner of this page in the space provided. There will be a space for a Co-Applicant to do the same.

Step 32 – Under the heading “Additional Information,” there will be two questions to be answered by checking the box marked “Yes” or the box marked “No.” (the first question will ask if you own a waterbed while the second will ask if you will permit smoking on the leased property).

Step 33 – Under the words “Please Answer,” will be 9 questions focused on your past and any extraneous hardships in either your finances or residences. Both applicants will need to place a checkmark in either the box marked “Yes” or the box marked “No” for each question. Note if you have checked “Yes” for any of these question, you must provide an explanation. (You may attach a sheet to this application to divulge a full explanation.”

Step 34 – Under the heading “Debts,” name any Loans you may be responsible for, the Name of the Creditor you owe, the Balance you are responsible for, and the Monthly Payment you are responsible for.

Step 35 – Beneath the heading “Assets,” list any type of Asset you hold along with its value (you will need to submit supporting paperwork for this).

Step 36 – In the table labeled “Other Occupants of the Premises,” you must report anyone who will be living with you on the leased property. In the table provided list the Last Name, First Name, and Middle Initial in the first two columns. Then indicate if they are Male or Female, Date of Birth, and Relationship this person holds with you.

Step 37 – The last four lines are reserved for the name and contact information of any emergency contacts you may have available. For each one enter his/her Name, Relationship to you, Email address, Telephone Number, Street Address, City, State, and Zip Code.
