The North Carolina Rental Application Form is a document that is used by owners, landlords and management companies as an initial step in the determination as to whether or not an applicant may be a good fit for tenancy for their property. The prospective tenant would complete the application and return it to the appropriate representative for their review. If the landlord is comfortable with the prospective tenant after review of the application, then the tenant would be offered the opportunity to take residency on the property. All information must be typed or printed.

How To Write

Step 1 – To begin, enter the name of the community where the potential tenant may wish to reside

  • Date the application in mm/dd/yyyy format
  • Enter the apartment and or address of desired occupancy
  • Enter the expected occupancy date

Step 2 – Applicant’s Information –

  • Name of Applicant
  • Home Phone
  • Work Phone
  • Date of Birth
  • Social Security Number
  • Driver’s License Number
  • State (NC)
  • Enter all of the same information for a co-applicant, if any

Step 3 – Answer the following questions by checking yes or no at the end of each question:

  • Are you or your spouse a current, illegal user of a controlled substance -If the answer is yes -explain on the lines provided on the form
  • Have you or your spouse ever been convicted by a court of law? If your answer is yes, explain on the line provided on the form
  • Have you ever been convicted for any other felony offense? If your answer is yes – explain on the line provided

Step 4 – Occupants –

  • Enter the total number of persons who will occupy the apartment to include the applicant
  • Enter the name, age, date of birth and relationship to applicant(s)
  • Enter the information regarding who should be contacted in the event of an emergency
  • Mailing address of emergency contact
  • Telephone number
  • If you have pets specify the type, breed and weight

Step 5 – Residence History – Last 3 Years

  • Beginning with the current residence, followed by previous, enter:
  • Address
  • Landlord and/or mortgage company
  • Telephone Number
  • How long was the residency
  • How much was your monthly mortgage or rent payment

Step 6 – Applicant’s Employment History- Last 3 years

  • Enter your current employer first (name of company
  • Street Address
  • How long were you with the company
  • Monthly Income
  • Name of Supervisor
  • Telephone Number

Step 7 – Spouse’s Employment History- Last 3 years

  • Enter your current employer first (name of company
  • Street Address
  • How long were you with the company
  • Monthly Income
  • Name of Supervisor
  • Telephone Number
  • Source
  • Amount per month (provide documentation)

Step 8 – Bank References –

  • Checking Bank
  • Account Number
  • Telephone Number
  • AND
  • Savings Bank
  • Account Number
  • Telephone Number

Step 9 – Transportation –

  • Vehicle type and color
  • Tag number
  • County
  • State
  • Enter the same for any other vehicles you may park on or around the property

Step 10 – Application Fee -Carefully review the paragraphs

  • Enter the amount of the application fee
  • Read carefully Applicant’s  Release and Authorization
