A patient sign-in sheet allows a hospital, clinic, or other healthcare institution to record some identifying information regarding their visiting patients. Implementing the use of this document will allow for the smooth operation of any healthcare institution, ensuring that patients have arrived on time and are looked after accordingly.
We have also designed a sign-in sheet which allows for the recording of additional patient information; the patient sign-in sheet (extended) has space for doctor’s notes as well as a place to indicate whether or not the patient is a first time visitor.
How to Write
Step 1 – Download the Form
The patient sign-in sheet can be downloaded in PDF, ODT, and Word formats. Choose your desired format by selecting one of the links found within this paragraph or below the form image on the right of your screen.
Step 2 – Date and Patient Name
You should begin by recording the date at the top of the form. When the patient enters the medical office, have them print their name in the first column.
Step 3 – Appointment Information
The arrival time and appointment time can be recorded either by yourself or by the patient (depending on the policies of the medical office). Once that information has been supplied, write in the remaining column the name of the applicable doctor.
Step 4 – Presenting the Form
It’s best to present the form to patients at the reception desk so that they can record their arrival time directly after entering the medical office. You may also benefit from printing multiple copies of the form to ensure that you don’t run out of space for each patient’s information.
