Alabama Voter Registration Form – Register to Vote in AL

An Alabama voter registration form, known as “Form NVRA-2,” may be completed in the printable version and mailed-in or filed online. This is strictly for residents for the State of Alabama and one will need a residential mailing address in order to successfully complete the process. The deadline for signing up is fifteen (15) days before the date of election. If you chose to use the printable form, it must be sent to the Board of Registrars in your County.

Voter ID (§ 11-46-4) – In order to vote, a resident must have one (1) of the following State Approved ID’s.

How to Write

Step 1 – Download in Adobe PDF.

Step 2 – Registrants must answer “Yes” to questions (1) and (2) before continuing. Selecting “No” will generate a void form. Next, provide the following information under sections (3) through (8):

  • Full name
  • Former name (if reporting a name change)
  • Date of birth
  • Phone number
  • Email address
  • Alabama driver’s license number or other ID number
  • Current address of residence
  • Current mailing address
  • Previous address

Step 3 – Under sections (9) through (13), enter the following information:

  • Sex
  • Race
  • Place of birth
  • Map of location (if no street number or name was provided)
  • Name, address, phone number of person who assisted registrant (if applicable)
  • Signature
  • Date

Step 4 – Registrants must submit completed voter registration forms to their local County Board of Registrars.
