Florida Voter Registration Form – Register to Vote in FL

A Florida voter registration form must be submitted by all Florida residents who wish to vote in any upcoming election. Those in search of alternative registration methods can complete the online voter registration process through the “RegisterToVoteFlorida” website. Only Florida residents eighteen (18) years of age or older may register to vote in the State of Florida. All first-time Florida registrants must attach a copy of a valid photo ID or document proving identity (acceptable IDs and documents are mentioned at the top of the voter registration form). All registration forms and online submissions must be submitted twenty-nine (29) days before any local, state, or federal election.

Voter ID (§ 101.043)- All voters must present valid photo identification including a signature. A second piece of identification will be required for photo IDs without a signature.

How to Write

Step 1 – Download the Florida Voter Registration Form.

Step 2 – Select the type of registration; “New”, “Update”, or “Replacement Request Voter Card”. Provide the following information in sections (1) through (7):

  • U.S. citizenship
  • Ability to vote (non-convicted felon)
  • Ability to vote (not mentally incapacitated)
  • Date of birth
  • Florida driver’s license or Florida ID card number
  • Last 4 digits of SSN (if no ID number was provided)
  • Full name
  • Address

Step 3 – Next, enter the following information where applicable in sections (8) through (11):

  • Mailing address (if different from address provided above)
  • Previous registered address (if applicable)
  • Former name (if applicable)
  • Gender
  • State or country of birth
  • Phone number (optional)
  • Email address (optional)
  • Party affiliation
  • Race/ethnicity

All registrants must sign and date the form under section (12). First-time registrants must attach a photocopy of a piece of photo identification or a document providing proof of identity.

Step 4 – All registration forms can be submitted by mail or in-person to any Supervisor of Elections Office. Other authorized registration locations include offices that issue driver’s licenses or voter registration agencies.
