An Illinois voter registration form is a document that is used to enable a resident of Illinois to vote in any upcoming election. One must be at least seventeen (17) to vote in a primary election, or eighteen (18) if they wish to vote in a Federal election . Illinois residents may choose to complete the registration process online in lieu of submitting the registration form. Identification is required when registering to vote but, once proof of identity has been provided, voters will not be asked to show ID at the polling stations. Illinois residents must ensure that forms are submitted before the twenty-seventh (27th) day prior to an election.

Where to Send Voter Registration Form – Forms can be sent to the Elections Office within the registrant’s jurisdiction.

Voter ID (§ 5/5-105)- Voters are only required to present ID at the polling station if they are registering through Election Day Registration or a first-time voter who registered through the mail without providing proof of identity.
