Illinois Voter Registration Form – Register to Vote in IL

An Illinois voter registration form is a document that is used to enable a resident of Illinois to vote in any upcoming election. One must be at least seventeen (17) to vote in a primary election, or eighteen (18) if they wish to vote in a Federal election . Illinois residents may choose to complete the registration process online in lieu of submitting the registration form. Identification is required when registering to vote but, once proof of identity has been provided, voters will not be asked to show ID at the polling stations. Illinois residents must ensure that forms are submitted before the twenty-seventh (27th) day prior to an election.

Where to Send Voter Registration Form – Forms can be sent to the Elections Office within the registrant’s jurisdiction.

Voter ID (§ 5/5-105)- Voters are only required to present ID at the polling station if they are registering through Election Day Registration or a first-time voter who registered through the mail without providing proof of identity.

How to Write

Step 1 – Download the Illinois Voter Registration Form (Adobe PDF).

Step 2 – The registration form must be completed in its entirety in order to be valid. To begin, select “yes” or “no” to the following questions (selecting “no” to either question will result in a void registration form):

  • Are you a citizen of the United States of America?
  • Will you be 18 years of age on or before the next election day?

This form can be used to register to vote, to change registration address, or to change registration name; select the appropriate checkbox above section (1) of the form. You can also request to vote by mail in the November 3rd, 2020 general election. Select “yes” or “no” to complete this section of the form.

Step 3 – Next, enter the following information into sections (1) through (11):

  • Full name
  • Address of residence
  • Mailing address
  • Email address (optional)
  • Former registration address (if applicable)
  • Former name (if applicable)
  • Date of birth
  • Sex
  • Phone number (optional)
  • Identification number (driver’s license, State ID, or SSN)
  • Signature
  • Date

Complete section (12) if the registrant had help completing this form.

Step 4 – Forms can be mailed to the Elections Office within the registrant’s jurisdiction. Alternatively, registrants can visit their elections office or one of the following locations to register in person:

  • County clerk’s office
  • Board of election commissioner’s office
