Oregon Voter Registration Form – Register to Vote in OR

An Oregon voter registration form can be used by residents that would like to register to vote in an upcoming election. The document can be downloaded, filled out, printed off, and submitted to the resident’s local County Elections Office as an alternative to registering online. Both registration methods require a valid Oregon ID or a copy of one (1) of the approved state-approved IDs which shows the name and address of the applicant. In addition to the required ID, applicants must be at least seventeen (17) years old at the time of registration and at least eighteen (18) years old on election day. Confirmation of a successful registration will be received via a Voter Notification Card.

Where to Send Voter Registration Form – County Elections Office

Voter ID – Oregon is a vote-by-mail state which means voters needn’t show identification on election day. However, to register to vote in Oregon, applicants must provide a valid Oregon driver’s license, permit, or ID number. If no valid Oregon ID is available, the last four (4) digits of an SSN or a copy of one (1) of the following forms of identification will be accepted:

  • Valid photo identification
  • Paycheck
  • Utility bill
  • Bank statement
  • Government document
  • Proof of eligibility under the UOCAVA (Uniformed and Overseas Citizens Absentee Voting Act) or the VAEH (Voting Accessibility for the Elderly and Handicapped Act)
