An Illinois Do Not Resuscitate order form (DNR) ensures that medical providers do not attempt CPR on an individual should the need arise. Illinois has integrated the DNR into the all-encompassing Practitioner Orders for Life-Sustaining Treatment (POLST) form. Patients should use the POLST form as it is now the more widely accepted order.
