The Illinois General Bill of Sale a legal document that would usually be used for the sale, purchase or trade of any personal property between two private parties. This will serve as proof that the item(s) have been properly sold and purchased and will prove the change of ownership. This document will require notarization. How To Write

Step 1 – Seller and Buyer Information – Enter the following information on the form :

  • The Seller –

  • Driver’s License Number

  • City
  • State
  • AND
  • The Buyer –
  • Name
  • Driver’s License Number
  • Physical Address
  • City
  • State

Step 2 – Property Details – Enter a description of the property being sold and purchased on the line in the form as follows:

  • Description
  • Serial Number (SN) (if any)

Step 3 – Trade/Purchase Price – Check the boxes that apply and provide additional information (if needed):

  • If the first box is checked, the seller will accept only monetary payment – Enter the worded dollar amount
  • Enter the dollar amount in numbers
  • Specify the date as to when the parties have agreed that payment will be due and made payable, in mm/dd/yyyy format
  • Enter the date in which the document will be known, in mm/dd/yyyy format
  • If the parties have agreed that payment will be made payable at a later date, enter the date –  mm/dd/yyyy format
  • Other – If different negotiations have been agreed upon by the parties, enter a description of the final acceptable terms (if additional room is needed add a sheet and attach it to the form
  • Check the box if the item(s) are being provided as a gift to the “buyer”
  • The last box in this section will address a trade. Enter a brief description of the agreed terms of the trade

Step 4 – Seller’s Disclosure – The seller must read all statements in this section and enter any defects that may be known in the line provided. Once that is completed, if the seller is before a notary public, they may sign in the “Seller’s Signature” line

Step 5 – Signatures – Provide the following information as shown on the form:

  • Date the signatures in dd/mm/yyyy format at the top of this section
  • The Seller –
  • Signature
  • Printed Name
  • AND
  • The Buyer –
  • Signature
  • Printed Name
  • AND
  • The Witness – 
  • Signature
  • Printed Name

Step 6 –  Notarization – As the notary has identified all of the signing parties by current ID and witnessed all signatures, the notary will then complete the remainder of the form, and shall affix their state seal.

  • Make copies of the completed document to provide to all who signed for their records
  • Give the original copy to the buyer
