A waxing consent form allows a salon or spa to provide waxing treatments and to remain free of liability by providing clients with information regarding any post-waxing skin sensitivity or side effects. This form also provides written notification of the possibility of skin sensitivity/thinning caused by the waxing treatment. The client, being made fully aware of the ramifications, accepts the responsibility of these possible results by signing the document. If the client is under the age of 18, a parent or guardian will have to sign the consent form.
