2 – The Vehicle Owner Must Report The Attorney-in-Fact’s Identity

The first blank line on this form requires the Legal Name of the Attorney-in-Fact entered. Make sure this is identical to the I.D. issued by the KS DOT. 2 – The Vehicle Must Be Clearly Designated As The Subject Of This Authority

Below the required designation of Principal Authority statement will be four blank lines. Enter the Year, Make, and Style of the Vehicle using these three blank spaces. Then on the blank line labeled “ID” number, enter the Vehicle Identification Number as it appears on the Vehicle’s Title/Registration.

3 – Report Any Existence Lienholder

If any loans or financing has been involved in the Principal’s attainment of this Vehicle, it must be reported. There will be enough room to identify two Lienholders. You may leave this section blank if it is inapplicable, however, if any lienholders are relevant to this Vehicle’s Ownership, enter their Names and Addresses on the blank lines provided (1st Lienholder, 2nd Lienholder). If there are more than two, attach a sheet of paper with the rest of this roster.4 – The Verification Of This Document May Only Occur Through The Principal Vehicle Owner’s Signature

Beneath the statement beginning with “By my signature…” the Vehicle Owner granting power must Print his or her Name on the blank space labeled “Hand Printed Owner(s) Name,” then Sign his or her Name on the blank space labeled “Signature of Owner.” The Date this signing occurred must be entered just above the word “Date” by the Vehicle Owner at the time of signing.
