A media consent and release form is a legally binding document between a releasor who consents and waives their right to any claims for audio and video recordings made by someone else (released party). Its use can be for promotion or publication on TV, in print, or online. The consenter must be at least 18 years of age to authorize.

What to Include

  • Consenter’s Name. The person who is consenting to have their audio, video, and likeness presented for commercial or personal use.
  • Released Party’s Name. The party that will own the media and its right to use and publish.
  • Media Types. To publish specific media types such as photographs, video recordings, and audio recordings.
  • Right to Edit. The right to alter and modify the media for the purpose of promoting the content.
  • Compensation ($). If the consenter is being compensated, this should be mentioned. If no compensation is being offered, the media’s promotion will act as consideration.
  • Release and Waiver. A statement by the consenter that releases any claims they may have to the media.
  • Representation. The consenter claims to understand the contents of the release and that, being at least 18 years of age, they are in a position to authorize the form.
