A Vermont Lease Termination Letter Form (60 or 90 Day Notice) is a notice that is served to a tenant in a month-to-month lease arrangement in order for either the landlord or tenant to terminate their rental contract. The notice is sent with sixty (60) days if the tenant has been on the premises for two (2) years or less, or ninety (90) days if the tenant has been residing for more than two (2) years on the property.

How To Write
Step 1 – Contact Information – Enter:
- Date the Notice in mm/dd/yyyy format
- Enter the address of the premises to be vacated
- Select the 60 or 90 day option to vacate
Step 2 – Notice Information –
- The party who will send notice must enter the name of the person that they are sending notification to
- Select the appropriate box stating whether you are the landlord or tenant providing notice
- If the tenant is providing notice, also provide an address where the security deposit may be sent
Step 3 – Service –
- The person serving the notice must complete the Proof of Service and collect signature if possible
