  • Are either the primary applicant and/or their spouse, a full or part time student? (Indicate by selecting and checking the appropriate answer)

  • What is the full time hourly required by the educational institution either party (or both) is/are attending?

Step 12 – Qualifying Questions – (answer yes, no and enter the state if necessary)

  • Have you or your spouse ever been convicted of a crime? (if yes, enter the state)
  • Will there be any other occupants over the age of 18, other than those that are listed?

Step 13 – Fees

  • Landlord must enter the amount of the deposit and what amount will be non-refundable as an application fee
  • Enter the amount that will be refunded to the applicant(s) should their application be rejected

Step 14 – Signature(s)
