A daycare sign-in sheet allows daycare workers to keep track of the arrival and departure times of children under their supervision. Since the form requires that the child’s parent or guardian provides their signature when dropping off and picking up their child, it will allow daycare workers to record the name of the individual with whom the child arrived and left. Step 1 – Download the Form

Step 2 – Initial Information

Begin by recording the date when the child is to arrive. When the parent/guardian shows up with their child, you should write down the child’s name as well as the time in (time of arrival). Have the parent attest to this information by providing their signature.

Step 3 – Departure Information

When the parent/guardian returns to pick up their child, record their time out (time of departure). The parent/guardian should once again provide their signature to verify this information.
