Massachusetts Notary Acknowledgment Form

A Massachusetts notary acknowledgment form is a document signed by a notary public to officially verify a signature on a legal document. The notary acts as a witness to the signature of the document in order to certify that the signer has signed of their own free will and is not under any duress. Once the notary confirms the signer’s identity, they will notarize the document by providing their signature and seal on the notary acknowledgment form.

Find a notary public near you. You should be able to find a notary at one (1) of the following locations:

Once you have found a notary public, make an arrangement to meet them and present yourself at the appointed time.

3. Show ID

Identify yourself and show the notary your photo ID.

4. Sign

Sign the document in the notary’s presence if it has not been signed already.

5. Seal

The notary will fill out, sign, and provide their seal on the notary acknowledgment form.

Verify a Notary in Massachusetts

Massachusetts does not currently offer an online notary verification service. In order to obtain information about a notary, contact the Public Records Division in the Secretary of the Commonwealth’s Office.

Alternately, some counties have the ability to provide details about notaries commissioned by them. Contact the county clerk’s office to see if they can be of assistance.

How to Write (For Notary)

Step 2 – Provide the name of the county you are in, the date, your name, and the name of the document signer.
