A Pennsylvania notary acknowledgment form is used to authenticate the signing of a document. It indicates that all parties mentioned within the document were present at the time of signing and that their signatures are in fact their own. This form will often be required for estate transfers, vehicle titles, firearm bills of sale, and other documents which must be authenticated by an official. The notary must identify the individual(s), based on personal knowledge or valid identification, and confirm their signature(s); they may sign the document in the presence of the notary or declare that a previously recorded signature is indeed their own. Once the notary has confirmed the identity and signature of all parties, they may proceed by completing the acknowledgment form and returning the initial documents back to the principal individual(s).

How to Write (For Notary)
Step 1 – Begin by downloading the Pennsylvania Notary Acknowledgment Form in PDF (.pdf), Microsoft Word (.docx), or Open Document Text (.odt).
Step 2 – Specify the name of the state and county in the first two (2) available fields. Below that, enter the date (day, month, year), name of the notary, the name of requesting an individual, and mention the name of the individual once again in the remaining space.
Step 3 – To complete the form, the notary must provide their stamp, signature, and their official title.
