Pennsylvania Notary Acknowledgment Form

A Pennsylvania notary acknowledgment form is used to authenticate the signing of a document. It indicates that all parties mentioned within the document were present at the time of signing and that their signatures are in fact their own. This form will often be required for estate transfers, vehicle titles, firearm bills of sale, and other documents which must be authenticated by an official. The notary must identify the individual(s), based on personal knowledge or valid identification, and confirm their signature(s); they may sign the document in the presence of the notary or declare that a previously recorded signature is indeed their own. Once the notary has confirmed the identity and signature of all parties, they may proceed by completing the acknowledgment form and returning the initial documents back to the principal individual(s).

How to Notarize in Pennsylvania (4 steps)

1. Locate Notary

In order to notarize a document, you must first locate a notary public operating within the State. The following locations typically offer notarial services:

The document which is being notarized should be filled in completely before being brought to the notary public. The Revised Uniform Law on Notarial Acts states that you may provide your signature on the document prior to notarization if, in the presence of the notary, you acknowledge that the signature is your own. However, if your document has yet to be signed, it is recommended that it remain unsigned until the notary requests your signature.

3. Present ID

Once the notary has reviewed your documents and confirmed your identity, they will request that you provide your signature on your documents or acknowledge that your signature is, in fact, your own (if the signature was already provided).

4. Seal

The notary will fill in the acknowledgment form and provide their seal/stamp to complete the process. At this point, your documents will have been authenticated and will be returned back to you.

Verify a Notary in Pennsylvania (4 steps)

Step 1

To verify a notary by name, scroll down to “Option 1” and enter the individual’s name in the empty fields. You may also specify their commission status and whether or not they are active. Click Find By Name to proceed.notary search by name

You may also search for a notary by specifying their commission ID. This can be accomplished by scrolling down to “Option 2” and entering their commission ID number in the empty space. Click Find By Commission ID to continue.field for searching notary by commission ID

Your results should be that of the single individual whose commission ID number matches the one supplied in your search.

Step 4

To search for all notaries within a specific location, navigate to “Option 3” and enter an address in the empty spaces. You may also specify whether you’d like to search for notaries of all commissions statuses, and for notaries who are active. Proceed by clicking Find By Location.notary search by location

How to Write (For Notary)

Step 1 – Begin by downloading the Pennsylvania Notary Acknowledgment Form in PDF (.pdf), Microsoft Word (.docx), or Open Document Text (.odt).

Step 2 – Specify the name of the state and county in the first two (2) available fields. Below that, enter the date (day, month, year), name of the notary, the name of requesting an individual, and mention the name of the individual once again in the remaining space.

Step 3 – To complete the form, the notary must provide their stamp, signature, and their official title.
