An Arizona medical power of attorney is a document that allows a family member or spouse to handle the medical decisions of a patient should the patient become incapacitated. A medical power of attorney can be canceled at any time by the principal completing a revocation form.

“Health care power of attorney” means a written designation of an agent to make health care decisions that meets the requirements of section 36-3221 and that comes into effect and is durable as provided in section 36-3223, subsection A.[2]
