An alcoholics anonymous (AA) sign-in/attendance sheet (for group leader) is a form that a group leader or chairperson can use to record the attendance of their meetings. In addition to simply documenting the names of those in attendance, the form also has space for attendees to provide some basic personal information (i.e. phone number, email address). Note that those in attendance may not wish to give their personal details, so be sure to specify that their personal information is requested and not demanded.

How to Write

Step 1 – Download the Form

The form can be downloaded in multiple formats (PDF, ODT, and Word). Download the file in your preferred format and open it with the correct viewing application on your computer.

Step 2 – Group Information

As the group leader/chairperson, you will need to fill out the meeting information at the top of the page. This can be accomplished by specifying the group name, location of the meeting, and your name.

Step 3 – Member Information

The attendee should receive the form and begin by specifying the date. Alternatively, you can record the date yourself to streamline the process. Following that, have each attendee supply their name, phone number, and email address.

Step 4 – Presenting the Form

The form should be printed and distributed to each attendee either at the start of the meeting or after it has finished. Remember, they may wish to not specify their personal information as they may be in attendance solely to support a member or to have a look at how a meeting is run, which is common for open speaker meetings and open discussion meetings.
