Ohio bill of sale forms describe the details of the sale of a piece of private property. They are important for the seller and buyer to maintain records if there are any questions regarding ownership of the property in the future. These forms can be used for any property but are most commonly used to transfer vehicles and boats.

BMV Bill of Sale – This document would be utilized to provide proof of a legal sale, purchase, and change of ownership of a motor vehicle and is likely to be required at registration.

Vehicle Registration Forms
- Bill of Sale;
- Odometer Disclosure Statement (Form BMW-3724);
- Certificate of Title (apply for duplicate titles at the local deputy registrar license agency);
- Valid driver’s license;
- Ohio Motor Vehicle Power of Attorney is required if the owner appoints another person to register the vehicle for them;
- Registration Fees; and
- Proof of Insurance with Minimum Requirements:
- $25,000 for the injury or death of one person (per accident);
- $50,000 for the injury or death of two or more people (per accident); and
- $25,000 for property damage (per accident).
Boat Registration Forms
- A completed Certified Watercraft Registration Application (DNR 8460R);
- A Bill of Sale;
- If the boat needs to be titled, a notarized copy of the original title signed over to the buyer, or for new vehicles purchased through a dealer, a manufacturer’s statement of origin. For a list of Ohio Title Offices where the title may be officially transferred, click here;
- Valid driver’s license or identification; and
