A North Dakota notary acknowledgment form is a document attached to a legal instrument in order to verify the authenticity of the signatures found therein. This type of notarization provides certification that the individual who signed the document is who they say they are and that they signed the document of their own free will. Furthermore, the notary must determine that the document is bonafide and that the signer is of sound mind. If the notary public approves the document and the signer’s identification checks out, the notary will fill out and sign the notary acknowledgment form. Once the notary has filled out the form and added their valid notary stamp or seal, the document will be considered notarized.

How to Write (For Notary)
Step 1 – Download the North Dakota Notary Acknowledgment form in one of the three (3) formats listed below.
Step 2 – On the form, you need to supply the following information into the indicated spaces of the form:
- Name of your associated county
- The date (day/month/year)
- Name of person acknowledging
- Name of person giving an oath (if person acknowledging is incapable of presenting themselves)
Step 3 – Next, you will have to provide the following:
- Your handwritten signature
- Your printed name
- Your title or office
- Your commission expiry date
