North Dakota Notary Acknowledgment Form

A North Dakota notary acknowledgment form is a document attached to a legal instrument in order to verify the authenticity of the signatures found therein. This type of notarization provides certification that the individual who signed the document is who they say they are and that they signed the document of their own free will. Furthermore, the notary must determine that the document is bonafide and that the signer is of sound mind. If the notary public approves the document and the signer’s identification checks out, the notary will fill out and sign the notary acknowledgment form. Once the notary has filled out the form and added their valid notary stamp or seal, the document will be considered notarized.

Locate a notary who can provide you with a notary acknowledgment and make an appointment with them. Notaries can often be found at one of the following:

Make sure that your document is completely filled out and that you have a notary acknowledgment section or form attached. When making your appointment with a notary public, verify their requirements for a notary acknowledgment.

3. Bring ID

Bring your document to the notary public and show them a valid piece of state-issued identification. If the document is unsigned, sign it in front of the notary public. Declare that your document was signed of your own free will.

4. Acknowledgment

Once the notary is convinced that your identity is authentic, they will notarize your document by filling out the notary acknowledgment form.

Verify a Notary in North Dakota (3 steps)

Step 1

To conduct an online search for a notary in North Dakota, navigate to the Notary Search page of the North Dakota Secretary of State’s website.

Step 2

Enter either the notary’s name or commission number before clicking Search to view your results. Clicking the Advanced tab allows the search to be refined if necessary.

How to Write (For Notary)

Step 1 – Download the North Dakota Notary Acknowledgment form in one of the three (3) formats listed below.

Step 2 – On the form, you need to supply the following information into the indicated spaces of the form:

  • Name of your associated county
  • The date (day/month/year)
  • Name of person acknowledging
  • Name of person giving an oath (if person acknowledging is incapable of presenting themselves)

Step 3 – Next, you will have to provide the following:

  • Your handwritten signature
  • Your printed name
  • Your title or office
  • Your commission expiry date
